Looks great. Only question is whether or not the zip code is really supposed to end in 0000... the vendor said it was when comparing to real physicals, and I doubt any kind of bouncer or clerk would notice.
There is some white space between the background and the head in the picture but is barely noticeable. I also sent it an already photoshopped picture so nothing to complain about here.
